Initial Visits/Forms
On your initial visit you will be asked to fill out the new patient forms to the right. Please feel free to print them off and fill them out ahead of time.
On Your First Visit:
- Download, print and fill out the new patient forms to bring with you on your initial visit. (The forms are located to the right.)
- Bring your insurance card (if applicable).
- Make sure you do not brush your tongue the day of your appointment.
- Ensure that you are hydrated and have eaten prior to your treatment.
- Finally, wear comfortable clothing.
